Commenting on HMIE's report into services to protect children, Liberal Democrat Children's spokesperson Margaret Smith MSP said:
"Given high profile failings in child protection, like the tragic death of Brandon Muir, it's alarming that a quarter of HMIE inspections revealed serious weaknesses in child protection services.
"Although HMIE is quick to point to subsequent improvements, there are still areas where urgent progress is needed.
"If Scottish Ministers are serious about improving child protection, then they must address the weaknesses identified in the report.
"This should start with urgent action to improve the assessment of risks and needs of vulnerable children as almost half of the areas assessed were evaluated as weak or unsatisfactory."
